"First, for your new first name, you take the first three letters of your real first name and add the first two letter of your real last name."
"Second, for your new last name, you take the first two letters of your mother's maiden name and add the first three letters of the name of the city where you were born."
"Then, for your honorific name and title, you take the last three letters of your real last name and reverse them. Then add the name of the first car you drove/owned. Insert the word &aops;of' and tack on the name of the last medication you took."
